One thing missing from the recent Republican efforts to reform Obamacare is what George H.W. Bush called “the vision thing.” What would a Republican health care system look like? How would it differ from the Obamacare exchanges the Democrats have created?

For example, in a real market wouldn’t insurers be free to offer a wide variety of products to meet the varied needs of a diverse population? Why should teetotalers be forced to buy coverage for substance abuse? Why should women who have gone through menopause be forced to buy coverage for maternity care? Why should men have to buy coverage for mammograms? Why should women have to buy coverage for prostate cancer?
